| Welcome to our online
self-activation system. If you purchased your prepaid medical discount
card at a retail store, you may activate your membership by entering your
membership number below and completing the membership form. Please enter your
membership PIN #, found on the back of your card or on your register receipt,
in the space below.
Our system updates your purchase at the retail location, by the next business day. You will not be able to activate your membership until after we have updated your payment in our system. If you need immediate assistance, please contact our Customer Service department at (866) 635-9532 for immediate activation. |
